Grant Contract Forms and Information
Successful business grantees are required to submit the following forms with the grant application. These documents will become part of the grant project contract.
- No Overdue Taxes Certification
- Conflict of Interest Policy Template - place the text from this template on company letterhead and modify as needed. Or submit a copy of your organization's internal Conflict of Interest Policy
- Non-Debarment Form
