For accidents involving property damage only, the employee's supervisor should be promptly notified. Supervisors should contact the Health and Safety Officer to determine the level of investigation needed.

For accidents involving Motor Fleet Management (MFM) vehicles, complete form FM-16 and contact MFM at 919-733-6540 (M-F 7am to 5pm). After hours call State Capital Police at 800-277-8181. MFM staff will walk the employee through the insurance process.
